How to Qualify:
Families may qualify for our program by meeting at least one of the following criteria. Selection also depends on slot availability.
Temporary Assistance for Needy Families (TANF)
Supplemental Security Income (SSI)
Supplemental Nutrition Assistance Program (SNAP)
Homeless
Foster Care
OR
Low Income
Verification of all information provided on Waitlist Application required upon selection.
Steps to get on our Waitlist & Enrollment
Complete the online waitlist application below
Receive a call from our Early Childhood Outcomes Assistant regarding waitlist status.
Receive a call from a Family Service Worker upon selection to go over enrollment/eligibility.
If selected, scheduled enrollment will take place with family service worker and the following documentation will be required. (list out documentation)
Proof of eligibility (SSI,TANF/SNAP, homeless verification, foster placement agreement, proof of income and previous taxes…)
Child’s birth certificate, Medical (insurance) card, Well Child assessment, dental assessment, up to date immunizations, custody or legal documents, and list of family/friends authorized to pick up child with their government names, address, birthdays and phone number.
